Financial responsibility documents have not been submitted to the EHD for 2013 year. Last <br />financial responsibility documents received were dated on April 2012. Current financial responsibility <br />documents are required to be submitted annually. Complete and submit a copy of the financial <br />responsibility either to EHD or electronically on CERS. <br />113. Designated operator Tim Adams, exp 050815, and Sam Delgado, exp 02315, have been <br />performing monthly inspection at this facility and notification was not provided to the EHD within 30 days <br />of the change. Tim performed a monthly inspection in August 2013. Sam has been performing monthly <br />inspection since November 2012 to June 2013. EHD received notification from Walton Engineering, <br />dated 07/31/13, signed by Tank Owner representative Dave Nelson, stated that Walton Engineering <br />employees would be performing monthly reports. No monthly reports produced by Walton Engineering <br />were found on site. Any changes shall be reported to the EHD with 30 days. Submit notification to the <br />EHD or electronically on CERS identifying all the designated operators for this facility. <br />204. The designated operator failed to visually inspect the UST system for leaks, water, or debris <br />during the September and July 2013 monthly inspection. Tomorrow is the last day left in October 2013 <br />and no report is produced as of today. During the monthly inspection, the designated operator shall <br />visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which <br />there is no record of a service visit. Ensure that designated operators performing monthly inspections at <br />this facility are inspecting all of the required areas and noting it on the on the reports. <br />205. The September and July 2013 designated operator monthly inspection report were not found on <br />site. Tomorrow is the last day left in October 2013 and no report is produced as of today. Designated <br />operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br />ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site.
