The record-keeping section of both tanks, 87 and <br /> 91, listed"No" for equipment maintenance and calibration records, but they should be yes. The training section for <br /> both tanks, 87 and 91, listed"No"for designated operator training, but they should be yes. The 91 MLLD manufacturer <br /> is listed as"FE/Petro Redjackent" and the MLLD model is listed as"R,J. FXIV"when the 91 MLLD manufacturer <br /> should be listed Redjacket and the MLLD model should be FXIV. The 91 piping secondary containment was not <br /> indicated when it should be listed as dry. The monitoring plan must be uploaded to the California Environmental <br /> Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. This was corrected on site. <br /> This is a minor violation. <br /> 125 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on February 24, 2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by March 26, 2015. The <br /> following violation was not addressed: <br /> #207: Designated Operator did not inspect all required items. <br /> This information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Please request for a <br /> copy of inspection report that issued on February 24, 2015, if needed. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br /> the EHD to add violation item #125. The amended report replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork,within 30 days of receiving the amended inspection report. <br /> ***NOTES FROM THE PREVIOUS ISSUED INSPECTION REPORT <br /> Notes: A verbal consent to inspect, photograph and sample was given by Aman Mehroke, Owner. All violations <br /> observed during the inspection were corrected on site.
