A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log <br /> into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload a copy of the <br /> UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> According to EHD records, a cold start was performed on 12/9/2017 by Elite IV as part of an ISD upgrade for a <br /> dispenser replacement(SR0078124).The cold start was not approved as part of the permit application for the <br /> dispenser replacement. EHD must be notified of all permit changes prior to commencing work. Immediately submit <br /> a revised permit detailing the scope of work that was performed along with documents showing the testing of the <br /> sensors. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator,Alex Jabbari, failed to document all the alarms from the attached alarm history on the <br /> 4/18/2017, 5/22/2017, 11/22/2017 designated operator monthly inspection reports and failed to check that they were <br /> responded to appropriately. <br /> The missing alarms include: <br /> 4/18/2017 report: L2 Regular Fill Sump Fuel Alarm on3/24/2017 <br /> 5/22/2017 report: L2 Regular Fill Sump Fuel Alarm on 4/16/2017 <br /> 11/22/2017 report: all of the alarms(L1-L6)received a sensor out alarm and a fuel alarm on 1/13/2017. <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class 11. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by March 15, 2018. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
