The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control District for <br /> applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br /> commencing work. <br /> 3. At least one person who holds the International Code Council"UST Installer/Retrofitter" <br /> certification must be onsite AT ALL TIMES when work is performed. <br /> 4. A "Certificate of Compliance for Underground Storage Tank Installation Form C" shall be <br /> submitted by the owner or the owners'agent [pursuant to California Code of Regulation(CCR) <br /> 2635(d)] before this upgrade will be considered complete by SJCEHD. <br /> 5. Contact Steven C. Shih at(209)468-9850 to arrange the following inspection appointments; a 48- <br /> hour notice for inspection appointments is required. <br /> a. Corrosion protection: prior to backfilling around the spill buckets, corrosion protection of <br /> the risers must be verified. <br /> b. Overfill prevention: overfill prevention system in compliance with CCR 2635 (b)(2)must be <br /> verified. <br /> c. Spill Bucket: after concrete was poured, spill bucket with minimum of 5 gallon capacity and <br /> a 30-minute lake test in compliance with CCR2635(b)(1)must be verified. <br /> d. Third party tank integrity tests must be performed once concrete is poured,prior to the <br /> contractor signing off of the jobsite. The test results must be submitted to SJCEHD within 7 <br /> days after the test. <br /> 6. Permit expires 90 days from the approval date.
