The missing alarms include: L3 Supreme Annular Space Fuel Alarm and a Q2 Regular PLLD <br />Shutdown Alarm on February 21, 2014 . During the monthly inspection, the designated operator shall review the <br />alarm history for the previous month, check that each alarm was documented and responded to appropriately, and <br />attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by March 19, 2015 . <br />Consent to perform the inspection, take photos and collect samples was given by Kim Castillo the Manager. This <br />facility's designated operator is Elijah Pendergraft (ICC expiration date: 6-2-16 ) . The service technician was <br />Dave Mathie (ICC expiration date: 8-26-16 ), who had the following manufacturer's certifications: VR, 9-10-16, <br />VMI, 1-4-17, Tank Tester, 9-30-16. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if <br />applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br />Compliance. <br />NOTES: <br />*Verified all sensors placed at lowest point <br />*Before start of inspection, the service technician tested the 87 spill bucket and realized that the drain valve was <br />not functioning properly. The technician place a bladder ball inside the spill bucket and tested the spill bucket. The <br />test passed. Wayne Perry replaced the drain valve on the 87 spill bucket.
