Please send the following documents <br />to the EHD to complete the application. The permit application is not approved at this tine. <br />1. On page three, section four of the permit application, write in brand names and part <br />numbers for all UST system parts being installed including penetration fittings, fitting <br />adhesive a sealant, spill container park and spill contame "' <br />2. Provide third party approval letters for all UST system park being installed Including <br />penetration fittings, fitting adhesive or sealant, spill container parts and spill containers- <br />Cut <br />o tainers.Cut sheets which state "third party approved" are rot approval letters. <br />3.
