The Diesel Master line leak detector failed to detect a leak when tested. All line leak detectors shall <br />be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, <br />trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) <br />and obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br />immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br />18. The 87 unleaded spill container failed when tested. All spill containers shall have a minimum <br />capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br />Immediately contact a properly licensed, trained, and certified contractor to repair or replace the spill <br />container under permit and inspection of the EHD. <br />Submit the return to compliance certification form with a corrective actions statement and supporting <br />documentation by October 24, 2010. <br />Note: The secondary containment test is due in/or before January 2011. All test results are required to <br />be submitted within thirty days of the test to the EHD. The certification financial responsibility <br />documents had been emailed the EHD previously and not received. The documents were provided <br />while on site. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS ILITY IS SUBJECT TO REINSPECT ON AT AN TIME AT EHD'S CURRENT HOURLY RATE. <br />EH Inspect <br />Re i e <br />e: I n p <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
