Probe out <br /> alarms occurred on this tank on 9/25/17. According to the designated operator monthly inspection report and the site <br /> operators, this alarm was caused when Walton Engineering returned a few days after the last monitoring system <br /> certification to correct an issue with the probe floats. These records shall be maintained on site for at least three <br /> years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration <br /> and maintenance records; (3) results of any visual observations", (4) results of sample analysis performed a lab or in <br /> the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test <br /> results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance <br /> and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class 11. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 18, 2018. <br /> "The monitoring system certification portion of the inspection was conducted on 9/17/18 and was not witnessed. <br /> The sumps were inspected prior to testing - proper sensor location was verified, no liquid was observed, and tank <br /> form and monitoring plan information was verified. The routine paperwork inspection was conducted on 9/18/18. <br /> The inspection report was provided at that time. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
