Yosemite Avenue,Manteca <br /> The proposed plan and application for permit to install new underground storage tanks <br /> at the above referenced location, have been approved with the following conditions: <br /> 1. Please provide PHS-EHD with the name of the contractor to be used prior to <br /> commencing work. The list provided with the permit includes Savidge <br /> Construction which does not have an active contractor's license. The contractor <br /> chosen must have an active license and active worker's compensation insurance. <br /> 2. The tank system will be installed with pressurized piping, not suction piping. If <br /> suction piping is to be used,additional submittals will be required. <br /> 3. All new submittals supercede the previously submitted ones. <br /> 4. As builts of project must be provided before final permit to operate will be <br /> granted. <br /> 5. Submit state form C for each tank system to EHD upon job completion. <br /> 6. Automatic pump shutdown must satisfy all conditions listed in Title 23 2636(8) <br /> of CCR or annual line tightness testing will be required. <br /> 7. All inspections to be scheduled at least 48 hours in advance. <br /> 8. Please contact the local Fire Department for permitting requirements. <br /> 9. Attachments are part of the plan and must accompany plans on site. <br /> 10.Any changes in the information provided must be submitted to EHD for approval <br /> prior to commencing work. <br /> 9. The tanks will require hold downs as specified in the bouyancy calculations. <br /> 10.The overspill buckets for the product fill must be a minimum of 5 gallons and <br /> allow free product to drain into the tank. <br /> I I.Please contact the City of Manteca for approval of the oil/water separator and <br /> clarifier. <br /> 12.This permit expires at the end of the calendar year. <br /> Please contact me at (209) 468-9848 to schedule inspections and with any questions. <br /> A Division of San Joaquin County Health Care Services <br />
