The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Muniappa Naidu at(209) 468-3439 to arrange the following inspections <br /> appointments (a 48-hour notice is required for inspection appointments): <br /> a. Leak Detector Test: Leak detector must be tested to confirm that the <br /> device can detect a leak of 3 gallons per hour at 10 psi. <br /> b. Spill Bucket: Verify minimum 5-gallon capacity and pass a 60-minute <br /> Lake Test or equivalent. Proper drainage or removal must also be <br /> verified. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced. <br />
