The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact John Alaniz at (209) 468-3147 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br /> monitoring has been restored to meet the requirements of Title 23 California Code of <br /> Regulations and the Health & Safety Code. A copy of the alarm history shall be printed <br /> prior to performing the "cold start" of the Veeder Root system and made available at the <br /> time of EHD inspection. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. If concrete will be broken near a tank, a secondary containment test must be performed on the <br /> affected LISTS once concrete is poured in accordance with the manufacture's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> 1868 E.
