Hammer Lane <br /> Stockton, CA 95219 <br /> Your application for a permit, SR0050245, to break concrete and replace the 87 octane spill <br /> bucket and possibly replace the drop tube at the above referenced address has been received and <br /> approved with the following conditions: <br /> 1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for <br /> any applicable permits. <br /> 2. Any changes shall be submitted to this office for approval prior to commencing work. All <br /> new addenda supercede the previous ones. <br /> 3. At least one person who holds a current International Code Council (ICC) UST Service <br /> Technician certification and manufacturers' certificate of training for all components <br /> installed or tested must be onsite when work is performed. <br /> 4. Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 5. Contact EHD for the following inspections at least 48 hours in advance: <br /> a. Corrosion protection on the fill riser <br /> b. Verification that the new spill bucket holds a minimum of 5 gallons and pass a 60 minute <br /> lake test or equivalent after concrete is poured. <br /> c. If the drop tube is replaced, EHD must verify the overfill prevention system is in <br /> compliance with CCR 2635 (b)(2). <br /> 6. Complete and submit a copy of a UST Installation Certificate of Compliance(Former Form <br /> C) for the 91 octane tank no later than 30 days after work is completed. <br /> 7.
