The permit has <br />been approved with the following conditions. <br />Conditions of approval: <br />1. Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supercede <br />the previous ones. <br />3. Before the alarm history is cleared by the "cold start", a current printout must be <br />created and available for EHD inspection. Inspection may occur during the monitoring <br />system functionality test. <br />4. If the under dispenser containment (UDC) pans are being modified to add electrical <br />penetration fittings and the electrical penetrations are not significantly higher than the <br />product line penetrations, an approved test must be performed to determine the integrity <br />of the UDC pans. <br />5. Contact Garrett Backus (209-468-2986) to arrange the following inspection(s) (a 48-hour <br />notice is required): <br />a. The EHD must witness the testing of the functionality of the monitoring system <br />to ensure that monitoring has been restored to meet the requirements of <br />California Code of Regulations Title 23 and the Health & Safety Code. <br />b. The EHD must witness testing of the UDCs, if necessary. <br />6. If concrete is being broken near the UST system, pipeline and tank integrity testing must <br />be performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br />guidelines. Results must be received by the EHD within seven days of returning the <br />facility to operating condition. <br />Page 1 of 2 <br />
