Complete one form for each tank. <br />Certification of Financial Responsibility and Letter from Chief Financial Officer: <br />Complete the forms provided, and make a copy for your records. This document is <br />required to be submitted to this office annually. <br />Underground Storage Tank Monitoring and Response Plans: Complete and submit a <br />copy. Indicate in Section IX of the Monitoring Plan the type of overfill prevention <br />present for each tank. Retain a copy onsite for future reference by employees. <br />Report #5021: Update all information for all programs at this site including the UST and <br />Hazardous Waste programs, sign and date. <br />The owner of the tanks must submit a notification form identifying the "Designated UST <br />Operator" for each facility owned. A notification form is enclosed. <br />7. Go to: hqp•//www waterboards.ca. og v/cwphome/ust/re ug latoM /index.html to obtain the <br />current statutes (Underground Storage of Hazardous Substances) and UST Regulations <br />that you are required to have for your monitoring and response plan. <br />Also enclosed for your information is a brochure of Best Management Practices (BMPs) <br />for Retail Gasoline Outlets that you can use as part of your BMPs for your facility. <br />Hazardous Materials Management Plan: Contact the Office of Emergency Services at <br />468-3969 for information on updating this plan. <br />00.Information and instructions for obtaining a permanent California EPA ID number: <br />Complete the application for a California Hazardous Waste Permanent ID Number and <br />fax it to (916) 255-4703. <br />
