HEALTH AND SAFETY CHECKLIST FOR INITIATING FIELD WORK AT <br /> JOHN DEERE LANDSCAPES (FORMER LESCO FACILITY) <br /> PRE-MOBILIZATION REQUIREMENTS FOR ALL FIELD TASKS: <br /> Contact H&S Coordinator(Mike Quillici)to discuss scope and schedule of upcoming field work. <br /> Determine whether tasks are covered under existing ERM JHAs. <br /> Confirm that all employees have adequate/appropriate training for the upcoming work. <br /> If subsurface work will be conducted,has an experienced person been assigned? <br /> Determine whether work permits will be necessary(hot work,confined space,etc.). <br /> Hold a pre-field kickoff meeting with ERM field staff,project manager,task manager,and SSO. <br /> PRE-MOBILIZATION REQUIREMENTS FOR WORKING WITH SUBCONTRACTORS: <br /> Confirm that subcontractor is pre-qualified and meets all minimum ERM H&S requirements. <br /> Include an explanation of ERMs H&S program and requirements(daily tailgate,PSCs,equipment <br /> inspection forms,etc.)in the request-for-bid. <br /> F] Send text and figures section of the HASP to subcontractor for their employees to review before beginning <br /> work. All site workers must sign the HASP before work can begin. <br /> Request that the subcontractor provide ERM with copies of H&S certifications for all employees who will be <br /> on site,including current medical,40-Hour HazWOPER,and current 8-Hour HazWOPER refresher. If any of <br /> these items are missing,that employee will not be permitted to begin work. <br /> ElRequest that the subcontractor provide ERM with copies of all applicable JHAs for the their tasks. <br /> These JHAs must be reviewed by the task manager and field staff before beginning work. <br /> FIRST DAY OF FIELDWORK: <br /> Conduct tailgate meeting with ERM field crew,subcontractor field crew,task manager,and SSO to review and <br /> sign HASP,discuss work that will be performed,and conduct a site walk with all parties present. <br /> ® SSO will check in with field crew at least once during the day. <br /> DURATION OF FIELDWORK: <br /> Field copy of HASP must be kept with ERM field crew at all times. <br /> Tailgate meetings must be conducted before beginning work each day. <br /> SSO will make periodic visits(approximately 2 to 3 short visits per week depending on duration of field work) <br /> All site workers must fill out PSCs daily. <br /> ERM field personnel will review PSCs at the end of each day and discuss all safety events with the field crew <br /> during the next morning's tailgate meeting. <br /> SSO will collect PSCs and enter into ECS on a weekly basis. <br /> POST-FIELDWORK: <br /> eUpdate files with completed H&S documentation. <br /> Conduct a brief lessons-learned/follow-up meeting with ERM field staff,task manager,and SSO. <br /> 0121271/0121289 Updated 9/29/2011 <br />
