5.0 HAZARD IDENTIFICATION AND CONTROL <br /> 5.1 JOB HAZARD ANALYSES <br /> Prior to initiating any new project activity, or when there is a change in site <br /> conditions, the FSO will assist project team members in completing a JHA. <br /> The JHA will list the hazards associated with the project activity as well as <br /> associated control strategies. JHAs for the tasks listed in Section 4.1, as <br /> well as a blank copy of the JHA form,are located in Appendix B. As new <br /> task-specific JHAs are developed, they will be attached to this HASP. All <br /> necessary forms associated with task-specific JHAs are available in the field <br /> office trailer on the H&S table. <br /> 5.2 SUBSURFACE CLEARANCE <br /> Prior to initiating any ground disturbance activities,project team <br /> members will follow the ERM SSC Process. An SSC Experienced Person <br /> will administer the SSC process from start to finish. The SSC Process Flow <br /> Chart, the SSC Requirements and Procedures Checklist, and the SSC <br /> Procedure Field Checklist are included in Appendix D. <br /> 5.3 SITE INSPECTIONS <br /> The FSO or designee will inspect the job site at least once per week (or <br /> once per day during large-scale construction activities)using the Site <br /> Inspection Checklist in Appendix E as a guide. Completed checklists will <br /> be submitted to the HSC and retained in the site safety file. <br /> 5.4 BEHAVIOR-BASED SAFETY <br /> Everyone on site as art of this project will make a commitment to work <br /> Y p p ) <br /> safely and to look out for others on the jobsite. ERM has developed a <br /> Personal Safety Contract(PSC) as a tool to help ERM personnel and ERM <br /> Contractors think about the safety-related aspects of the work at hand, as <br /> described in applicable JHAs. <br /> Prior to the start of each work day,workers will fill out the appropriate <br /> sections of a PSC. By completing their FSCs each day, site workers agree <br /> to make a daily commitment to their own safety and the safety of those <br /> around them.
