r <br /> Dole Fresh Fruit <br /> 8751 Hwy 12, Victor <br /> Page 2 <br /> An error was made when the substance was.listed on the "Notification of Hazardous <br /> Waste Discharge" and the "Unauthorized Release Form" (UAR);gasoline should have <br /> been listed. The date notified, August 8, 1999, on the forms is the date the PHS/EHD <br /> staff made the determination that a release from the UST had occurred which may have <br /> impacted groundwater. The date discovered on the UAR is the date PHS/EHD staff was <br /> notified that the stockpiled soil had been placed in the former UST excavation. There <br /> was no further site evaluation until Dole Fresh Fruit requested a"no further action <br /> required" letter from the UST unit of PHS/EHD. The site was referred to the Site <br /> Mitigation unit of PHS/EHD for evaluation because the contaminant levels in the <br /> stockpiled soil placed back in the UST excavation exceeded levels allowed by the "site <br /> closure evaluation" form used by the UST unit staff. <br /> For the reasons stated above, further investigation of the vertical and lateral extent of <br /> contamination at this site is required.
