INTRODUCTION <br />' At the request of Mr Charlie Jacobs,Advanced GeoEnvironmental, Inc (AGE) has prepared this <br /> monitoring well installation and quarterly report for the property located at 13889 West Highway 12, <br /> Lodi, California The site and surrounding area are illustrated on Figure 1 Structures, soil borings, <br />' and monitoring well locations are illustrated on Figure 2 Site background information is <br /> summarized in Appendix A <br />' 2.0. PROCEDURES <br />' Field work was performed in accordance with guidelines issued by the Central Valley Regional <br /> Water Quality Control Board(CVRWQCB)for the investigation ofunderground storage tank(UST) <br /> sites and by the San Joaquin County Environmental Health Department(EHD) for the sampling of <br /> Iground water monitoring wells The scope of work was outlined in the AGE-prepared Monitoring <br /> Well Installation Work Plan Addendum, dated 13 September 2004 and approved by the EHD in a <br /> letter dated 24 September 2004 <br /> I2 1 MONITORING WELL INSTALLATION, DEVELOPMENT, AND SURVEY <br /> On 01 November 2004,four ground water monitoring wells(MW-1 through MW-4)were installed <br /> at the site The monitoring wells were developed on 03 November 2004, surveyed on 24 November <br /> 2004, and sampled on 29 November 2004 <br />' 2 1 1 Drilling and Sampling <br /> I On 01 November 2004, a CME-75HT truck-mounted drill ng was utilized to advance pilot soil <br /> borings MWI. through MW4 to 25 feet below surface grade(bsg)using 8 25-inch diameter hollow- <br /> stem augers The drill rig and two-man crew were supplied by Mitchell Drilling Environmental of <br /> I Rancho Cordova, California Auger returns generated by drilling were containerized in properly <br /> labeled,Department of Transportation(DOT)-approved 55-gallon drums and stored on-site pending <br /> proper disposal <br /> ISoil samples were collected ahead of the drill bit using a California modified, 18-inch split-spoon <br /> sampler loaded with three pre-cleaned, 2-inch by 6-inch brass sleeves The samples were collected <br /> Ifrom the pilot borings at 5-foot intervals, beginning at 5 feet bsg, using a 140-pound hammer to <br /> advance the sampler The number of blows required to advance the sampler over each 6-inch <br /> increment of the 18-inch sampling interval was recorded on the boring logs After sample recovery, <br /> Advanced GeoEnvironrnental,Inc <br />
