SAN 10 A Q U I N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Petco Store#1291 2350 W KETTLEMAN LN, Lodi November 13, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 101 CCR 66262.12 Managed a hazardous waste without an identification number. <br /> OBSERVATION: This business does not have an active hazardous waste ID number in the California Department <br /> of Toxic Substances Control(DTSC)database. <br /> REGULATION GUIDANCE: A hazardous waste generator shall not treat, store, dispose of,transport or offer for <br /> transportation, hazardous waste without having received an Environmental Protection Agency(EPA)Identification <br /> (ID)Number.A State EPA ID number is required for generators of less than 100 kg RCRA hazardous waste per <br /> month, less than 1 kg of acutely hazardous waste per month or any amount of non-RCRA hazardous waste. ID <br /> numbers are unique to a business and site location. <br /> CORRECTIVE ACTION: Go to the DTSC web site at <br /> https://dtsc.ca.gov/apply-for-hazardous-waste-epa-id-number/to reactivate your State ID number. Immediately <br /> reactivate your ID number and submit evidence to the San Joaquin County Environmental Health Department <br /> within 30 days. <br /> This is a minor violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: Designated facility signed copies of the following Uniform Hazardous Waste Manifest(UHWM) <br /> numbers were not found on-site: <br /> -013120694FLE(6/6/2019)(generator copy not found onsite) <br /> -005888089FLE(4/18/2018)(see photo IMG 9381) <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in <br /> accordance with section 66262.23(a)for three years or until the generator receives a signed copy from the <br /> designated facility which received the waste.This signed copy shall be retained as a record for at least three years <br /> from the date the waste was accepted by the initial transporter. The hazardous waste generator must submit a <br /> copy of the manifest to the California Department of Toxic Substances Control(DTSC)within 60 days if the <br /> destination facility signed manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department(EHD). If the destination facility signed manifests cannot be <br /> found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br /> signed copy was not received by the generator.
