S0 JOA U 1 Environmental Health Department <br /> u ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Save Mart#386 1 1172 N Main St, Manteca June 03, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: The eyewash station located in the facility's receiving area did not have the tube connected to it, <br /> to allow used eyewash solution to flow from the station to a storage container. The eyewash station also did not <br /> have a storage container to collect used eyewash solution.Additionally,the eyewash station was missing the fill <br /> cap. Rick Miller, the store manager, attached the tube to the eyewash station and placed the other end of the tube <br /> into a container. Rick stated he will order a new fill cap for the eyewash station. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to <br /> assure its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the eye wash station into proper working order by repair, replacement or testing. <br /> Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br /> were: <br /> 1. Two hazardous waste containers, storing soaps and detergents,were missing the name of the generator on the <br /> hazardous waste labels. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: The violation was corrected during the inspection by Rick Miller,the store manager,who <br /> wrote the name of the generator on the hazardous waste labels. No further action is required to correct this <br /> violation. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 7/3/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
