Observation:A complete modified contingency plan was not posted at <br /> the facility. <br /> Initial Banfield Response: As outlined in Banfield's Waste Management Plan, all Banfield hospitals are required to <br /> maintain a Contingency Plan, consisting of identifying emergency coordinators and post required information near the <br /> phone.The name and emergency phone numbers of the emergency coordinator(Field Director), location of <br /> emergency/spill equipment, and the local fire department number/911 have been posted in the hazardous waste <br /> storage area for this hospital. A photograph of the posting is provided in Attachment C (Photo 1). <br /> Subsequent Agency Response:The modified contingency plan does not have the locations of spill control equipment <br /> and fire extinguishers. Revise the modified contingency plan and re-submit the plan. <br /> Subsequent Banfield Response:The location of the spill kit, fire extinguishers, and eye wash station have been added to <br /> the contingency plan that is posted in the hazardous waste storage area for this hospital.A photograph of the posting is <br /> provided in Attachment A (Photo 1). <br /> Violation#605: Failed to completely label containers or portable tanks of hazardous waste. Observation: Hazardous <br /> waste containers were observed on-site with incomplete labeling. <br />
