Box 5745 <br /> Fremont, CA 94537 <br /> RE: Quik Stop #120 IN REPLY REFER TO SITE CODE: 1261 <br /> 9321 Thornton Rd. <br /> Stockton, CA 95209 <br /> The San Joaquin County Public Health Services Environmental Health <br /> Division (PHS-EHD) has been included in the State Water Resources <br /> Control Board (SWRCB) UST Pilot Program for oversight of assessment <br /> and clean up activities at contaminated underground tank sites. <br /> The attached "Notice of Reimbursement" is official notification <br /> that your site has been assigned to this Pilot Program. Fees for <br /> review activities and inspections associated with contaminated site <br /> assessment activities will not be billed by this office. The SWRCB <br /> will invoice you directly for activities performed under the <br /> auspices of the UST Pilot Program. <br /> We are in receipt of the Work Plan and Site Safety Plan for the <br /> Preliminary Report as prepared by Exceltech. The permit <br /> application for the drilling of monitoring wells has been issued <br /> and the drilling date is scheduled to commence Thursday, June 28, <br /> 1990. <br /> As stated in your April 2 , 1990 Letter of Acknowledgment, the <br /> Preliminary Report completion date was May 15, 1990. In order to <br /> adequately define soil and/or groundwater contamination, the <br /> Preliminary Report should be submitted to PHS/EHD and the Central <br /> Valley Regional Water Quality Control Board (CVRWQCB) no later than <br /> September 10, 1990. The Preliminary Report will include field <br /> activities to date, a recommendation for identifying the full <br /> extent of soil and/or groundwater contamination, and include a time <br /> schedule for completion of the next phase of investigation. <br /> A Division of San Joaquin County Health Care Service '� <br />
