Report of Closure Activities 1 <br /> Barbara Drake 26485 North Thomton Road,Thornton,Califomia Aril 16 1998 <br /> 10 INTRODUCTION <br /> On behalf of Ms Barbara Drake, Fluor Daniel GTI, Inc (Fluor Daniel GTI)submits this Report of <br /> Closure Activities for the abandonment of three groundwater monitoring wells, sod stockpile sampling <br /> and the backfilling of the former underground storage tank (UST) excavation at the subject site, located <br /> at 26485 North Thornton Road, Thornton, California (Figure 1) All work was done in accordance with <br /> Fluor Daniel GTI's Work Plan for Well Destruction Soil Stockpile Sampling and Excavation Backfill, <br /> dated September 3, 1997, and approved by the San Joaquin County Public Health Services, <br /> Environmental Health Division (PHSIEHD) The groundwater monitoring wells were destroyed in <br /> response to the August 12, 1997 letter from the PHS/EHD, requesting abandonment of the wells take <br /> place prior to issuance by the PHS/EHD of a "No Further Action" letter for the site Scopes of work <br /> performed for each task are summarized below <br /> 20 ADDITIONAL ASSESSMENT WORK <br /> 21 Site-Specific Health and Safety Plan/Permitting <br /> Fallowing a complete review of site conditions, Fluor Daniel GTI prepared a site-specific Health and <br /> Safety Plan as required by the Occupational Safety and Health Administration (OSHA) Standard <br /> "Hazardous Waste Operations and Emergency Response"guidelines(29 Code of Federal Regulations <br /> 1910 120) The document was reviewed and signed by all Fluor Daniel GTI personnel and <br /> subcontractors prior to performing work at the site <br /> A well destruction permit was obtained from the PHSIEHD, and a backfill excavation certificate <br /> obtained from the San Joaquin County Community Development Department(CCDD) Copies of <br /> these documents are included in Appendix A <br /> 22 Well Destruction Procedures <br /> On September 19, 1997, Fluor Daniel GTI supervised the abandonment of groundwater monitoring <br /> wells MW-1, MW-2 and MW-3 (Figure 1) Well MW-1,which historically has not contained petroleum <br /> hydrocarbons above detection limits, was abandoned by employing a tremie-grout procedure The <br /> well box was dug out and removed, and an excavation dug such that the casing and well seal were <br /> 20700512 CLQ FWOR DANIEL GTI <br />
