Two black containers with white lids labeled as containing "most in one" hazardous waste (see photos IMG <br />1163-7). <br />2. A back container with a white lid storing silver nitrate stick waste, but labeled as containing hazardous waste <br />aerosols (IMG 1168-70). <br /> <br />- Located in the OR Holding area: <br />3. A black container with a white lid labeled as containing Gemcitabine (IMG 1180-3). <br />4. A black container with a white lid labeled as containing alcohol prostate wipe packets (IMG 1186). <br />- Located in the Pharmacy: <br />5. A black container with a white lid storing loose pills, medication/vaccine vials, and two used silver nitrate sticks <br />(IMG 1189-93). <br />- Located in the Environmental Services (EVS) storage room: <br />6. A black container with a white lid storing what appeared to be chemical containers (IMG 1195, 1198). <br />7. A black container with a white lid storing aerosol cans (IMG 1195-7). <br />- Located in the ED Soil Utility Room: <br />8. A black container with a white lid labeled as containing "vag spec" (IMG 1204-7). <br />- Located in the Laboratory: <br />9. A black container storing what appeared to be used chemical packaging and containers (IMG 1208-10). <br />10. A black container labeled as containing "acid/base containers, preservative" and "HCL, formalin, glycol, <br />methanol" (IMG 1208, 1211-12) <br />REGULATION GUIDANCE: A container holding hazardous waste shall always be closed during transfer and <br />storage, except when it is necessary to add or remove waste. <br />CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are <br />closed when not adding or removing waste.
