You were notified by PHS/EHD in correspondence dated August
28, 1991 that the presence of contaminants,at your facility exceed the allowable limits of leaching potential as
recommended by the 1989 LUFT Manual prepared by the State Water Resources Control Board (SWRCB). At that
time you were advised that PHS/EHD would be utilizing a prioritization system to evaluate your site. The results
from the groundwater samples collected at the time of the July 31, 1991 tank removal and the, groundwater
contamination evidenced at 2420 Grant Line Road, indicate the possibility of a co-mingled groundwater
contamination plume. Your site has been prioritized and placed into the local Oversight Program for the oversight of
1 assessment and clean up activities.
1 The attached"Notice of Reimbursement"is official notification that PHS/EHD's Underground Storage Tank Local
oversight Program has identified you as a responsible party for the above mentioned site. Fees for review activities
and inspections associated with contaminated site assessment activities will not be billed by this office. The SWRCB
will invoice you directly for activities performed under the auspices of the Local oversight Program. The goal of the
site evaluation performed by PHS/EHD is to certify closures of underground tank leakage sites. Closure certification
occurs only after mitigation activities am completed.
in addition, The"Regional Board Staff Reommmendations For Initial.Evaluation and Investigation of the Underground
Tanks"and*Appendix A7 reporting format and the LUFT Manual shall be Utilized as guidance documents in the
preparation of work plans and/or reports.
A work plan to initiate soil and groundwater investigation should be submitted within.90 days of the date of this
letter.
