I tried to darken up the scan I received from Starlite Reclamation, but I don't think it got any better. The generator
address is our corporate office in San Diego. 4805 Murphy Canyon Rd. They have the zip code wrong. It is 92123. The
phone number is my cell. I don't see any address on the manifest for the Stockton shop. If you would like I can contact
Starlite and see if they can darken and rescan the manifest
From:Alaniz,John <lalanizl@slgov.or >
Sent: Wednesday, May 6, 2020 8:34 AM
To: Steve Heinen <s.heinen@pacificsteelgroup.com>
Subject: RE: Manifest
CAUTION:This email originated from outside of the organization. The generator address is not legible and is too faint.
This is the last thing I need to close out the violation.
Thank You,
John Alaniz
REHS
San Joaquin County
Environmental Health Department
1868 E Hazelton Avenue
Stockton, California 95205
Phone: (209)468-3147
Fax: (209) 468-3433
Please note, my email address has changed to jalanizl@sigov.org.
From: Steve Heinen [mailto:s.heinen@pacificsteelgroup.com]
Sent:Wednesday, May 6, 2020 7:00 AM
To:Alaniz,John <lalanizl@slgov.org>
Subject: Manifest
John,
I was able to get this copy of manifest 011830177 from Starlite. Hopefully this explains the invoice I sent you without a
manifest for 2 barrels of flammable liquid. Our Fabrication manager sent me a copy of the pick up ticket for a months'
worth of scrap from the Stockton shop. As I mentioned, the chips from the threaders are drained in the machine, then
removed and put in with the rebar scrap. I included a picture our maintenance guy sent me to show the size of the
chips.
2
