4. Alleged Violation: Item# 118 CCR 66265.16 Failed to maintain complete hazardous waste <br />training records. The facility must immediately train all employees who work with hazardous <br />waste in compliance with the time frame and training materials required. The documentation <br />must include a list of training topics, date of training, job title, name of employee, written job <br />description (qualifications and duties), and employee signed (or certified) record that <br />documents the training. <br />• Facility's Response: Please see Table 1 Regulated Waste Training Requirements by Program <br />Job Title (Attachment D). This table provides the job title of the associates who work with <br />hazardous waste, their written job description (qualifications and duties), and the list of <br />training topics they must be trained on. <br />• Please see training records for RCRA 22106 Training (Attachment E). <br />• Please see training records for RCRA State Specific Training (Attachment F) <br />• Please see training records for Emergency Action and Contingency Plan Training <br />(Attachment G). <br />• Please see training records for DOT Hazardous Waste Shipper Training(Attachment H) <br />5. Alleged Violation: Item# 301 CCR 66265.31 Facility not maintained to minimize the release of a <br />hazardous waste. The violation was corrected during the inspection by cleaning up the broken <br />lamp, putting it into a hazardous waste container, and labeling it as hazardous waste and <br />cleaning up the soap spill. <br />• Facility's Response: No further action required to correct this violation. The violation <br />corrected at time of inspection. <br />6. Alleged Violation, Item# 303 CCR 66265.33 Failed to maintain all communication or alarm <br />systems, spill control, or decontamination equipment. The facility must bring the eyewash into <br />proper working order by repair, replacement, or testing. <br />• Facility's Response: Expired eyewash station was replaced on 3-2-2020. Please see picture of <br />new eyewash station (Attachment 1). <br />7. Alleged Violation: Item 605# CCR 66262.34(f) failed to completely label containers or portable <br />tanks of hazardous waste. The facility must immediately label the containers as required for <br />hazardous waste. <br />• Facility's Response: Please see document titled "Updated Waste Labels" (Attachment J) <br />8. Alleged Violation: Item 709 -CCR 66273.36 UWH failed to train employees and maintain records <br />on UW handling and emergency procedures. The facility must immediately provide employees <br />with UW training In the detail required. The facility must generate a training record with a list of <br />trained employees and the date of training. The facility must provide a corrective action <br />statement, the training record, a detailed description of the training, and supporting <br />documentation. <br />• Facility's Response: Please see training slides (22-28) of RCRA Module 22106 (Attachment <br />K).
