I reviewed the application for ground water <br /> cleanup. Joe iienao, also of this office, has been assigned to review the <br /> application from the discharge point of view and to administer the permit. <br /> The application is incomplete because it lacks information from the <br /> "Application Requirements" (enclosed) and because our file does not contain <br /> the remediation system proposal or a Problem Assessment Report (PAR) , as <br /> described in Appendix A of the Tri-Regional Recommendations (also enclosed) . <br /> We will process the application after we receive the following information: <br /> 1. An evaluation of land disposal options, including discharge to a <br /> municipal wastewater collection system, and justification for selecting <br /> a surface water disposal alternative. <br /> 2. Narrative and schematic descriptions of the proposed extraction, <br /> treatment, and disposal systems and an estimate of the breakthrough time <br /> for each carbon unit. <br /> 3. A map showing the location of the extraction well (s) and the flow path <br /> of the proposed discharge to a major river or lake. <br /> 4. A map showing water supply wells and surface waters within 0.5 miles of <br /> the site. <br /> 5. An operation plan describing general treatment system operations and <br /> maintenance procedures, process controls, monitoring and pumping rates. <br /> 6. Effects of the extraction system on the ground water gradient and the <br /> plume. <br /> 7.
