The permit has been <br /> approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District <br /> for any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work . All new addenda supersede the previous ones . <br /> 3 . Provide written notification to ust ( slgov . org to arrange the following appointments (a <br /> 48 - hour notice is required for inspection appointments) . <br /> • Verification of the dispenser footprints contained by the under dispenser containment <br /> sumps ( UDCs ) . <br /> • Visual inspection of the piping within the dispensers . <br /> • Sensor functionality : Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California Code <br /> of Regulations and the Health & Safety Code . A copy of the alarm history shall be <br /> printed prior to performing the " cold start" of the monitoring system and made <br /> available at the time of EHD inspection . <br /> 4 . If concrete will be broken near the tank , a secondary containment test must be performed <br /> on the tank once concrete is poured in accordance with the manufacturer' s guidelines . <br /> 5 . Any individual performing the work of a service technician must meet the following <br /> qualifications : <br /> a . Possess or be employed by a person who possesses one of several permissible valid , <br /> current contractor' s licenses issued by the Contractors State License Board ( may be <br /> issued to the installation company) , or a Tank Tester' s license issued to the individual <br /> by the State Water Board , and <br /> 1868 E .
