OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The 04/01/2020 DO report did not include the owner/operator name and date signed -The 10/15/2019, 8/20/2019, 7/22/2019, 6/20/2019, and 4/23/2019 DO reports did not include the owner/operator name -The 4/23/2019, 6/20/2019, 7/22/2019, 8/20/2019, 9/17/2019, 10/15/2019,11/13/2019, 12/11/2019, 1/8/2020, 2/5/2020,3/4/2020, and 4/1/2020 DO reports failed to mark Yes or No to the question "Have all individuals performing facility employee duties received the required facility employee training within the past 12 months?" (Section XI), and the The DO reports failed to write the zip code. The DO reports incorrectly stated that the overfill prevention was conducted on 04/10/2019 the current date was 3/10/2019. -The 06/20/2019 DO report was performed more than 30days after the previous inspection. Initial inspection performed on 4/22/2019, next inspection performed on 6/20/2019, 61 days between the two dates. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. OBSERVATION: Seepage was detected below the shear valve in under dispenser containment (UDC)# 1/2, 5/6 and 7/8 indicating a leak in the primary containment. REGULATION GUIDANCE: All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary.
