To date, this well has not been <br /> installed; therefore, thE EHD is directing you to complete the well installation within thirty days <br /> of date of this letter. bmit a Well and Boring Permit Application, Master File Record (MFR), <br /> and permit/inspection I ae of$488 to the EHD, and notify the EHD of pending fieldwork at least <br /> a week prior to the wel installation. <br /> Submit a sensitive re eptor survey report to the EHD by 22 January 2011, that includes <br /> surface water bodies schools, hospitals, daycares, etc., as well as domestic, municipal, <br /> industrial and other IIs within a 2,000-foot radius of this site. Contact the Department of <br /> Water Resources, ML nicipal water suppliers, and nearby industries for well information and <br /> submit any known coi struction details of the wells. Perform a physical search of the area to <br /> identify all wells, and show the location of all wells on a map, relative to the location of this <br /> site. <br /> Also, conduct and su mit a risk assessment that includes an evaluation of the hazards and <br /> risks to human health and the environment from residual petroleum hydrocarbon contaminant <br /> concentrations comp ired to Environmental Screening Levels (ESLs) (San Francisco Bay <br /> Regional Water Qu lity Control Board), and California Human Health Screening Levels <br /> (CHHSLs) (Californi Environmental Protection Agency Office of Environmental Health <br /> Hazard Assessment) to the EHD by 10 February 2011.
