The following is an itemized list of underground storage tank violations that have not been addressed for MORADA CHEVRON FAST N EASY#60* as of October 07, 2020. Open violations from May 28, 2020 inspection ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Open violations from April 11, 2019 inspection Violation#104-Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. The monitoring plans for both tanks incorrectly list the monitoring panel for the tank, piping, and UDC monitoring as a Veeder Root TLS 350.The panel on site is a Veeder Root TLS 350 Plus. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#109-"Designated Underground Storage Tank Operator Identification Form" not submitted within 30 days. A new designated operator Omero Garcia II was hired and has been performing monthly visual inspections. No notification has been provided to the EHD. Any changes in designated operatos shall be reported to the EHD within 30 days.
