The following is an itemized list of underground storage tank violations that have not been addressed for MORADA CHEVRON FAST N EASY#60* as of October 07, 2020. Open violations from April 11, 2019 inspection Violation#209-Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). The designated operator employee training for Zubair, Ehtishain and Israr was not performed or not current. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#210-No employee present that were trained in the operation and maintenance of the UST system byaDO. The 91 north STP sump sensor was not located to detect a leak at the earliest opportunity.The sensor was located on the side opposite the fuel piping and was placed over a foot south of the lowest point. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity.The sensor was relocated to the lowest point of the sump by the service technician.
