A J OA U I Environmental Health Department <br /> COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FLAG CITY CHEVRON 6421 CAPITOL AVE, LODI May 20, 2020 <br /> Other Violations <br /> 2010 1 See below Unlisted General violation ❑V ❑R ❑COS <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 103 HSC 25292.2, 25299.30 Failure to submit and maintain complete and current financial responsibility documents. <br /> OBSERVATION: Current financial responsibility documents were not found on site. <br /> REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained on <br /> site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a minor violation. <br /> 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the <br /> test. <br /> OBSERVATION: Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on 9/3/2019 and a test report was submitted on 12/9/2019 , two months late. <br /> REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. <br /> This is a minor violation. <br /> 201 CCR 2712(b)(1-2) Failure to maintain monitoring, maintenance, and follow up action records. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -The 2018 monitoring system certification test results were not found on site.. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years.
