SAN J O A R N Environmental Health Department <br /> i , �V <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STOCKTON MOBIL 3* 1 2358 E WATERLOO RD, STOCKTON April 02, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -Reports are not signed by the owner/operator. <br /> -The questions regarding spill containers and fill pipes were left blank. <br /> -The question regarding UDC sensor locations was left blank. <br /> -The questions regarding tank and line tightness testing was left blank. <br /> -Section V was left blank. <br /> -Items marked as a No were not documented in Section III. <br /> -CERS ID was left blank. <br /> -The question regarding follow up action was left blank. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> OBSERVATION: The designated operator inspections for the following months were performed more than 30 days <br /> after the previous inspection: <br /> - Initial inspection on 4/2/20, next inspection performed on 5/4/20 (32 days) <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> This is a minor violation. <br /> 210 CCR 2716(f)DO monthly reports not maintained on site for 12 months or for 36 months (after October 1,2018). <br /> OBSERVATION: The 11/11/19, 12/11/19, 2/7/20, 3/6/20, and 6/2/20 designated operator inspection reports were <br /> not found on site. Alarm histories for all of these dates were found in the designated operator binder. <br /> REGULATION GUIDANCE: Designated operator inspection reports prior to October 1, 2018 shall be retained on <br /> site for twelve months. Designated operator inspection reports on or after October 1, 2018 shall be retained on site <br /> for thirty-six months. <br /> CORRECTIVE ACTION: Locate and ensure that copies of the previous thirty-six months of designated operator <br /> inspection reports are maintained on site.
