PHS/EHD has provided the following summary of the agreements reached for your information. <br /> I <br /> 1) it was agreed that if possible MWl will be destroyed by grouting prior to backfilling of the excavation. <br /> PHS/EHD will be notified prior to initiating field work to locate the remainder of MW 1 and will evaluate j <br /> the well for destruction options. <br /> 2) A grading permit will be obtained from the City of Stockton prior to initiating backfilling of the <br /> excavation. Receipts for additional fill and the final ompaction report will be submitted to PHS/EHD. <br /> I <br /> 3) A replacement well for MWl will be installed underPHS/EHD permit and sampled. After a minimum of <br /> 30 days all four monitoring wells, MWl (replacemen) through MW4, will be sampled. <br /> 4) Should groundwater contamination not be detected, a closure summary report will be prepared and site <br /> closure requested. After PHS/EHD staff evaluation aid approval, the site will be presented at a Technical <br /> Review Committee (TRC) meeting for closure evaluation. <br /> 5) If the TRC concurs with the staffs closure recomme dation, the site will be submitted to the Central <br /> Valley Regional.Water Quality Control (CVRWQCB) for closure evaluation.
