Please submit a PHS/EHD monitoring well drilling permit application and $89.00 permit fee <br /> prior to drilling. <br /> Please submit reports, work plans and letters to both PHS/EHD and the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB) until the investigation and cleanup is adequate for closure.
