Should any <br /> E unresolved concerns be identified during this 30 day review period, a mutually agreed upon compliance <br /> date will be established between the Regional Board staff, PHS-EHD, and the responsible party. <br /> Unresolved concems shall be detailed in written correspondence addressed to PHS-EHD by the <br /> Regional Board Executive Officer or his designated qualified Regional Board staff. The Regional <br /> Board shall forward a copy of all such correspondence to the State Water Board Contract Manager. <br /> You should have received all investigative documents relative to the remedial actions at this site. The <br /> most recent report prepared by Smith Environmental, "Closure Request Report" dated April 10, 1996, <br /> summarizes information regarding the site. A draft Case Closure Summary and a site chronology are <br /> enclosed for your information. <br /> Please provide your response no later than July 15, 1996.
