SANJ O 1` Q U I `IA,II, Environmental Health Department <br /> COUNTY <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> TOYOTA TOWN 2150 E HAMMER LN, STOCKTON March 05, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 120 HSC 25160.2(b)(4)Consolidated manifest receipts failed to contain complete information. <br /> The consolidated manifest receipts from Safety Kleen from January-August 2018 for used oil and used antifreeze <br /> were missing the shipping document portion. <br /> All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA)identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Provide copies of the Safety Kleen Shipping Documents and ensure that all future receipts for hazardous waste <br /> shipments using a consolidated manifesting procedure contain all required information. <br /> This is a minor violation. <br /> 512 CCR 66265.192(h) Failure to obtain an assessment or reassessment of a hazardous waste tank system. <br /> The tank assessment for the 450 waste oil tank and 250 gallon waste antifreeze tank was last done on January 15, <br /> 2010 and has not been reassessed. The owner or operator of a hazardous waste tank shall obtain a written <br /> assessment reviewed and certified by an independent, qualified, professional engineer, registered in California <br /> attesting that the system has sufficient structural integrity, is acceptable for the transferring, storing, and treating of <br /> hazardous waste, and that the tanks and containment system are suitably designed. Immediately have an <br /> assessment prepared for the 450 gallon waste oil tank and 250 gallon waste antifreeze tank. Submit a schedule <br /> showing when this will be accomplished to the EHD. <br /> This is a Class II violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> One empty black metal 35 gallon drum was observed in waste storage area without an "empty" label or the date it <br /> was emptied.
