Von Sosten Road <br /> Tracy, CA <br /> San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) has reviewed the "Wiork Plan Secondary Assessment <br /> and Compliance Monitoring" report dated October 17, 1991, prepared <br /> by Groundwater Technology, Inc. (GTI) . <br /> The monitoring well permit applications were not signed and a plot <br /> plan indicating which well(s) each permit application was for was <br /> not attached so the applications are being returned to GTT. They <br /> should be properly completed and returned with the $89. 00 permit <br /> fee for processing. Before applications for off-site work can be <br /> accepted a copy of the proper encroach6ent permit or property owner <br /> authorization must be submitted to PHS/EHD. <br /> The ground water sample results from the May 1991 sampling event <br /> have not been submitted to PHS/EHD.
