1 <br /> SAN JOAQUIN LOCAL HEALTH DISTRICT <br /> UNDERGROUND STORAGE TANK PROGRAM <br /> SITE CLEANUP ASSESSMENT PLAN REQUIREMENTS <br /> 1. Acquire the services of a California Registered or Certified Civil Engineer, <br /> Geologist, or Engineer Geoln,ist. <br /> 2. Submit a plan to determine the horizontal and vertical extent of contamination. <br /> The plan shall include the following: <br /> a. Name, address, and phone number of the consulting firm, property owner, <br /> facility operator, party responsible for cleanup and billing. <br /> b. The size and dimension of the parcel. <br /> c. Identify the type of material stored in the tank(s) and the amount of <br /> material released. (Audit of inventory records if available). <br /> d. Identify the depth of the water table and the normal regional groundwater <br /> -. gradient. <br /> e. Identify the so'; type as described by the Unified Soil Classification <br /> �.1 System, including subsurface soil identification if available. <br /> f. Identify the location and active status of all water wells : ,mated within <br /> V469-W")&—ft of the leak source. <br /> g. Identify the location of all underground storage tanks, septic systems, <br /> buildings, and underground utilities within 500 ft. of the leak source. <br /> h. Identify the location of all proposed groundwater monitoring dells and <br /> soil borings. <br /> i. Submit all available historical information on the existing underground <br /> storage tanks as to the age, type of construction, past repairs or <br /> replacement, existing monitoring wells or soil boring information. <br /> All Plan Assessment Reviews are subject to a $35.00 per hour consultation fee, <br /> plan check fee, monitoring well and soil boring permit fees. <br /> UGT 29 6/86 <br />
