Washington <br /> Stockton, CA 95203 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) <br /> has received correspondence from Unocal Corporate Environmental Remediation and <br /> Technology (CERT) dated January 6, 1993 stating there has been a change in <br /> management of the above referenced site. <br /> Although not the lead agency for oversight for this site, PHS/EHD issues permits and <br /> performs inspections for borings approved for the site by the Central Valley Regional <br /> Water Quality Control Board (CVRWQCB). As of April 3, 1991, Mittelhauser <br /> Corporation of Laguna Hills California has claimed themselves as the responsible party <br /> accepting billing for review of reports and site inspections associated with boring <br /> activities. <br /> Enclosed please find a copy of the signed "Site Mitigation Acknowledgment" (SMA) form <br /> from Mittelhauser and a blank SMA. <br /> With Unocal CERT now managing the site activities, a new SMA will need to be <br /> submitted to PHS/EHD before any boring permits can be issued. <br /> PHS/EHD requires a prepayment of review and inspection fees for sites under other lead <br /> agency oversight. Should proposals for further subsurface investigation be submitted to <br /> CVRWQCB in the future, a completed SMA form and a prepayment of $234 (3 hours @ <br /> $78 per hour) must be on file with PHS/EHD before a boring permit (separate $89 fee) <br /> can be issued. <br /> Please provide copies of all workplans and reports to PHS/EHD, as they are submitted <br /> to CVRWQCB for review and approval. <br /> A Division of San Joaquin County Health Care Services �0 <br />
