Gaitan, Jason
From: George Hamzik <George.Hamzik@dynatect.com>
Sent: Thursday, October 17, 2019 8:53 AM
To: Gaitan, Jason
Subject: Violation 608
Attachments: SJC Violations.pptx; SJC Violations.pdf
Hi Jason,
I have attached two files—both are the same presentation but did not know your preferred file format.
Violation 608:
Dynatect does not have 3 years of manifests because these filters have been accumulating for longer than 3
years. Therefore the drum was immediately identified as hazardous waste, specifically drained use oil and gas filters,
and disposed of under manifest. I am attaching pictures showing the labeling of the drum and the manifest for disposal.
This is documented on page 1 of the attached presentation.
Violation 605:
With respect to the eleven 5-gallon buckets, most were consolidated into 55-gallon drums and disposed of under
manifest. Page 2 of the attached document show the remaining 5-gallon buckets correctly labeled and page 3 shows
those consolidated drums being disposed of under manifest along with other waste liquids. In regards to the one 5-
gallon bucket with oily solids that was not labeled, that bucket along with several others were consolidated into a 55-
gallon drums and disposed of under manifest; page 4 of the attached. The 55-gallon drum with solids was labeled
legibly and disposed of under manifest; page 4.
In regards to the other issues:
• Two 55 gallon containers with used oil observed with no accumulation start date; labeled and disposed of under
manifest—page 3
• Five 55 gallon containers with rags contaminated with no accumulation start date; labeled and disposed of
under manifest—page 4
• Three 5 gallon buckets with used oil observed with no hazardous waste label; consolidated and disposed of
under manifest—page 3
• One 1 gallon bucket with used oil observed with no hazardous waste label; consolidated and disposed of under
manifest—page 3
In total five 55-gallon drums of waste oil was disposed of under manifest (page 3), and ten 55-gallon drums of oily solids
were disposed of under manifest (page 4).
1 am only showing one label for each waste stream since I do not have a group picture for the oil or solids. I hope the
labeling example and manifests are sufficient to show that things were labeled and disposed of correctly.
Please let me know if this is sufficient to close violations 608 and 605.
We are still working on violation 102—I will update you as soon as we have some tangible progress.
Regards,
George Hamzik
General Manager
1
