Current telephone number for State Office of Emergency Services <br /> (800-852-7550)is missing. The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non-sudden <br /> release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments,fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers(office/home/cell)of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all <br /> others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment,which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan,the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill <br /> the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br /> This is a repeat violation, Class II. <br /> 110 CCR 66265.54 Contingency plan not current. <br /> According to Mark Dufrene, Supervisor,the emergency coordinator, Ken Weeks, listed on the contingency plan no <br /> longer works at the facility.The contingency plan shall be reviewed and immediately amended, if necessary, <br /> whenever: <br /> -applicable regulations are revised; <br /> -the plan fails in an emergency; <br /> -facility changes in design, construction, operation, maintenance, or other circumstances that increases the <br /> potential for fires, explosions, releases of hazardous waste, or changes the response necessary in an emergency; <br /> -the list of emergency coordinators changes; or <br /> -the list of emergency equipment changes. <br /> Immediately amend the contingency plan, and submit a copy of the amended parts of the plan to the EHD for review. <br /> This is a minor violation. <br /> FA0004925 PRO507085 SCO01 08/08/2018 <br /> EHD 22-02 Rev.09/27/2018 Page 5 of 11 Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
