<br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System
<br /> (CERS)for 2018, 2019, and 2020. As of January 1, 2013, all businesses were required to submit all business plans
<br /> electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information,
<br /> and submit to the EHD for approval.
<br /> This is a Class II violation.
<br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page.
<br /> The Business Activities and Business Owner/Operator Identification forms have not been submitted for 2018, 2019,
<br /> and 2020. This information must be current and submitted as part of the business plan. Immediately log into the
<br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated
<br /> information, and submit to the EHD for approval.
<br /> This is a Class II violation.
<br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site.
<br /> The acetylene, oxygen, argon/carbon dioxide mixture, motor oil, waste oil, and used antifreeze have not been
<br /> reported for 2018, 2019, and 2020 and a 55 gallon drum of possible aluminum brightener (according to manager)
<br /> and at least 63 gallons of propane was found on site that has not been reported. Any material that meets or
<br /> exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California
<br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material
<br /> inventory, and submit to the EHD for approval.
<br /> This is a Class II violation.
<br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content.
<br /> A site map was not completed or submitted as part of the business plan for 2018, 2019, and 2020 and the last map
<br /> submittal was not legible and did not show/address loading areas, internal roads, evacuation staging area,
<br /> emergency response equipment (fire extinguishers and spill control), emergency shutoffs, and access/exit points,
<br /> and the locations of argon/carbon dioxide mixture, used antifreeze, motor oil, and propane. A site map shall contain
<br /> a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points,
<br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency
<br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as
<br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http-.Hcers.calepa.ca.gov/,
<br /> upload the correct or updated information, and submit to the EHD for approval.
<br /> This is a repeat violation, Class It.
