The following is an itemized list of underground storage tank violations that have not been addressed for QUIK STOP MARKET #3144 as of May 01 , 2020 . Open violations from July 18 , 2019 inspection Violation #124 - " Overfill Prevention Equipment Inspection Report Form " not submitted within 30 days of inspection . Overfill prevention testing was performed on 10/4/18 and a test report was not submitted to EHD . A copy of the test report must be submitted within 30 days of the tests . The correct date was 10/4/2018 . -The 10/01 /18 , 10/30/18 and 06/03/2019 inspection reports left blank the CERS ID and section VI . -The 11 /27/18 , and 12/26/ 18 inspection reports left blank the CERS ID , section V and VI . - The 02/19/2019 inspection report left blank the CERS ID and section VI . - The inspection reports of 04/8/2019 and 05/6/2019 was missing the CERS ID . - The inspection report of 07/01 /2019 left blank the CERS ID , section V and VI . During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report.
