PHS/EHD has <br /> not received a response from the CVRWQCB on our determination to date. <br /> i <br /> 'i. <br /> Monitoring Wells (MWs) remain at the site and must be destroyed under permit and inspection by <br /> PHS/EHD.'; Please submit a Monitoring Well Destruction Work Plan, PHS/EHD permit application and <br /> 560.00 permit fee to PHS/EHD by August 2, 1999. <br /> l <br /> In addition, pursuant to Section 25297.15(a) of the Health and Safety Code you must provide PHS/EHD <br /> certification in writing of your notification to all record fee title owners and a complete mailing list for all <br /> record fee;title owners prior to closure of the above referenced site.
