SAN JOHN Environmental Health Department Underground Storage Tank Program Inspection Report Facility Name: Facility Address: BILLS 76 Date: 633 E VIGTOR RD, LCDI February 19,2020 SUMMARY OF VIOLATIONS (CLASS I,CLASS II,or MINOR-Notice to Comply} Item# Remarks 202 CCR 2715(c)(3),2715(c)(4)Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). OBSERVATION: -The Facility Employee Training Certificates are not being filled in property. No initial training date is being documented. In the initial training date column,the current date of training is being documented. Several employees listed in training records were documented on numerous previous records,some going as far back as 2005. -The 9111119 training for Harinder Singh was documented on an outdated form. 1 REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1,2018,or within 30 days of hire if prior to October 1,2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION:Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 209 CCR 2716(a-e)Failure to conduct the designated UST operator visual inspection at least once every 30 days. OBSERVATION:The designated operator failed to visually inspect the STP sumps for leaks,water,or debris during the 1/24/20 and 11/27119 visual inspections. The reports state the alarms were caused by rain water entry and were not followed up by a service technician. REGULATION GUIDANCE:During the inspection,the designated operator shall visually inspect all spill containers, fill pipes,fill caps,and UDC sumps,and any tank top sump that had an alarm for which there is no record of a service visit,for damage,and for the presence of leaks,water,or debris,and check that the sensors are located properly. CORRECTIVE ACTION:Ensure that designated operators performing inspections at this facility are inspecting all of the required areas and noting it on the on the reports. This is a repeal violation,Class ll. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by March 20,2020, Starting September 1,2018, all in-office CERS help will be provided at EHD hourly rate($152).
