No initial training date is being <br /> documented. In the initial training date column,the current date of training is being documented. Several <br /> employees listed in training records were documented on numerous previous records,some going as far back as <br /> 2005. <br /> -The 9111119 training for Harinder Singh was documented on an outdated form. <br /> 1 REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1,2018,or within 30 days of hire if prior to October 1,2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION:Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e)Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> OBSERVATION:The designated operator failed to visually inspect the STP sumps for leaks,water,or debris during <br /> the 1/24/20 and 11/27119 visual inspections. The reports state the alarms were caused by rain water entry and were <br /> not followed up by a service technician. <br /> REGULATION GUIDANCE:During the inspection,the designated operator shall visually inspect all spill containers, <br /> fill pipes,fill caps,and UDC sumps,and any tank top sump that had an alarm for which there is no record of a <br /> service visit,for damage,and for the presence of leaks,water,or debris,and check that the sensors are located <br /> properly. <br /> CORRECTIVE ACTION:Ensure that designated operators performing inspections at this facility are inspecting all of <br /> the required areas and noting it on the on the reports. <br /> This is a repeal violation,Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by March 20,2020, <br /> Starting September 1,2018, all in-office CERS help will be provided at EHD hourly rate($152).
