The following observations were made in the facility's hazardous waste storage area: -A Toxic/Poison(blue)Hazardous Waste(HW)tote with accumulation start date 9/19/18 had a HW label that marked"over the counter medicines"and"electronics(e-waste)"as the contents inside the tote(see photo IMG 2284). However the following additional items were found in this tote: -A Loreal Infallible Mechanical Eyeliner marked"Used,"which scanned to be placed in the blue bin(IMG 2285-2287). Katrina Pierson,the Store Manager, included"eyeliner"as part of the contents on the HW label(IMG 2293). -A Daylogic Lavender Spray Lotion(IMG 2289). Katrina included"lotion"as part of the contents on the HW label (IMG 2293). -A broken Wet N Wild Pressed Powder,which scanned to be placed in the blue bin(IMG 2291-2292). Katrina included"makeup"as part of the contents on the HW label(IMG 2293). -A Corrosive/Base Alkaline(yellow) HW tote with accumulation start date 9/19/18 had a HW label that marked "aftershave,""laundry soap,"and"battery"as the contents inside the tote(IMG 2294). However,the following additional items were found in this tote: -Eucerin Daily Hydration Lotion(IMG 2295). -Neutrogena Triple Age Repair Moisturizer with sunscreen(IMG 2295). -Daylogic Antibacterial Body Wash(IMG 2295). -ORS Hair Mayonnaise(IMG 2295). -A Flammable(red) HW tote with accumulation start date 9/19/18 had a HW label that marked"all aerosols"and "makeup"as the contents inside the tote(IMG 2296). However,the following additional items were found in this tote: -A white out correction tape(IMG 2297). Katrina included"white out"as part of the contents on the HW label (IMG 2301). -A 2-pack of Rite Aid Home glue sticks(IMG 2297). Katrina include"glue sticks"as part of the contents on the HW label (IMG 2301). -A Secret deodorant(IMG 2297). Katrina included"Deod."As part of the contents on the HW label(IMG 2301). -Three bottles of nail polish(IMG 2297). Katrina included"nail polish"as part of the contents on the HW label (IMG 2301). All of the items listed above were identified as waste by Katrina at the time of inspection. The following was observed in the pharmacy: -When asked where the pharmacy keeps expired pharmaceuticals that are waiting to be sent back to Med Turn, Inc.,the pharmacy manager, Joy Orgi, pointed to an open unlabeled basket sitting on the floor of the pharmacy. This open basked contained a sealed bag labeled as"Control,"which contained several smaller sealed bags of loose damaged or crushed pills(IMG 2307-2309).According to Joy,these pills were damaged control pills,which could not be processed as hazardous waste because the pills were unidentifiable due to being damaged or crushed. However,the smaller sealed bags containing the pills appeared to be labeled with NDC numbers(IMG 2308-2309), and according to page 31 of Rite Aid's Retail Hazardous Waste computer-based training(dated 4/27/18), pharmacy hazardous waste includes"damaged or loose tablets or capsules that fall on the floor, broken tablets,etc." All hazardous waste containers shall be marked with the following information: -the words"Hazardous Waste" -name and address of generator FA0019801 PRO536064 SCO01 10/05/2018 EHD 22-03 Rev.09/22/16 Page 10 of 11 RCRA Large Quantity Hazardous Waste Generator OIR
