San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.si4ov.org/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> RITE AID#6510 4774 N WEST LN, STOCKTON October 05, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16(a-c) Failed to complete training on hazardous waste management and emergency response <br /> procedures. <br /> The following observations were made in the facility's hazardous waste storage area: <br /> -A Toxic/Poison(blue) Hazardous Waste(HW)tote with accumulation start date 9/19/18 had a HW label that <br /> marked"over the counter medicines"and"electronics(e-waste)"as the contents inside the tote(see photo IMG <br /> 2284). However the following additional items were found in this tote: <br /> -A Loreal Infallible Mechanical Eyeliner marked"Used,"which scanned to be placed in the blue bin(IMG <br /> 2285-2287). Katrina Pierson, the Store Manager, included"eyeliner"as part of the contents on the HW label(IMG <br /> 2293). <br /> -A Daylogic Lavender Spray Lotion(IMG 2289). Katrina included"lotion"as part of the contents on the HW label <br /> (IMG 2293). <br /> -A broken Wet N Wild Pressed Powder,which scanned to be placed in the blue bin(IMG 2291-2292). Katrina <br /> included"makeup"as part of the contents on the HW label(IMG 2293). <br /> -A Corrosive/Base Alkaline(yellow)HW tote with accumulation start date 9/19/18 had a HW label that marked <br /> "aftershave,""laundry soap,"and"battery"as the contents inside the tote(IMG 2294). However,the following <br /> additional items were found in this tote: <br /> -Eucerin Daily Hydration Lotion(IMG 2295). <br /> -Neutrogena Triple Age Repair Moisturizer with sunscreen(IMG 2295). <br /> -Daylogic Antibacterial Body Wash (IMG 2295). <br /> -ORS Hair Mayonnaise(IMG 2295). <br /> -A Flammable(red) HW tote with accumulation start date 9/19/18 had a HW label that marked"all aerosols"and <br /> ,'makeup"as the contents inside the tote(IMG 2296). However,the following additional items were found in this tote: <br /> -A white out correction tape(IMG 2297). Katrina included"white out"as part of the contents on the HW label <br /> (IMG 2301). <br /> -A 2-pack of Rite Aid Home glue sticks(IMG 2297). Katrina include"glue sticks"as part of the contents on the <br /> HW label(IMG 2301). <br /> -A Secret deodorant(IMG 2297). Katrina included"Deod."As part of the contents on the HW label(IMG 2301). <br /> -Three bottles of nail polish (IMG 2297). Katrina included"nail polish"as part of the contents on the HW label <br /> (IMG 2301). <br /> All of the items listed above were identified as waste by Katrina at the time of inspection. <br /> The following was observed in the pharmacy: <br /> -When asked where the pharmacy keeps expired pharmaceuticals that are waiting to be sent back to Med Turn, <br /> Inc.,the pharmacy manager, Joy Orgi, pointed to an open unlabeled basket sitting on the floor of the pharmacy. <br /> This open basked contained a sealed bag labeled as"Control,"which contained several smaller sealed bags of <br /> loose damaged or crushed pills(IMG 2307-2309).According to Joy,these pills were damaged control pills,which <br /> could not be processed as hazardous waste because the pills were unidentifiable due to being damaged or crushed. <br /> However,the smaller sealed bags containing the pills appeared to be labeled with NDC numbers(IMG 2308-2309), <br /> and according to page 31 of the Rite Aid's Retail Hazardous Waste computer-based training, pharmacy hazardous <br /> waste includes"damaged or loose tablets or capsules that fall on the floor, broken tablets, etc." <br /> Personnel who handle hazardous waste shall be trained within six months of assignment and take part in an annual <br /> review of the initial training received.
